IDENTITY CARD

Every candidate applying registration to the Executive Programme should also apply for IDENTITY CARD in the prescribed proforma (attached with the admission form - REG) which must be carried without fail for appearing in the ICSI examinations. The identity card proforma be filled up as per instruction given below -

     A recent passport size photograph is to be pased on the box provided in the proforma
            - The photograph pasted on the proforma should be signed by the candidate signature running beyond the breadth of the photograph

             - Candidate's photo and signature should be duly attested in the space provided for in the proforma beneath the column. Photo and Sgnature of the student attested.

              - Identity Card may be got attested by any of the following authorities with his / her seal carrying name, professional membership No., designation and complete official address

              1) Member of the Institute with ACS/FCS No.
              2) Gazetted Officer of the central or state govt.
              3) Manager of a Nationalised bank
              4) Principal of a recognized School / College
